Do I need a referral?
In Arizona Physical Therapists can treat patients without a physician’s referral. However, some insurances such as Medicare do require a referral for payment.
Do you take insurance?
Personalized Mobile Physical Therapy is a Participating Medicare Part B Provider. Have your Medicare card ready when you schedule.
What if I do not have traditional Medicare Part B Insurance?
Personalized Mobile Physical Therapy is considered out of network with commercial insurance plans. We can provide you documentation so that you may submit to your insurance for possible reimbursement if you have out-of-network benefits. It is the patient’s responsibility to check individual coverage for details.
Personalized Mobile Physical Therapy Self Pay Pricing:
Self pay rates are $150 per session. Each session is one on one with your personal physical therapist and lasts about an hour.
